"Root Cause " field should shld be added.
It will be good if "Issues" are supplemented by Root Cause for it. This will help in giving proper justification in while creating Metrics or say Client report.

since this can be achieve as a custom-field, I’m closing this request.

I have added 'root cause' as a custom field against the Issue entity (need admin access to do this) Doing this will also show an analysis by root cause in the dash board.
